You are the primary contact. An account will be created for you in order to manage classes, pay bills, etc. You still need to complete this section even if you are the student.

Privacy Statement

Colours of Dance is committed to the protection of privacy for each individual within its community, be that student, parent, teacher, employee or studio hirer. As part of this commitment, we will only collect, retain and process any data about an individual that is absolutely necessary to 
1 - providing the safe provision of dance tuition and examinations and
2 - communicating to teachers, students, parents, employees and studio hirers information about relevant activities in the school, special events and fees due.

Colours of Dance will not share any information about any individual with any third party without the specific, express and advance permission of the individual concerned. These Show Terms & Conditions explain how your information may be shared with Cambridgeshire County Council as required by them to process the Body of Persons Approval Licence should this be required.

Colours of Dance does not use ‘mailing lists’ during the performance preparation or at any other time. You will only receive performance emails from Colours of Dance administrative staff that are relevant to you/the student personally or to your class that is participating in the performances. There will be no paid for advertising in these emails.

For more detailed information about the collection and processing of personal information at Colours of Dance generally or specifically in referece to the performance participation, please contact the Principal.

You will now be asked to review the T&Cs below for the Filming Participation and must be comfortable being able to consent to these in order to participate in the performances.  Should you wish to review these at any time afterwards, you will be able to find a copy of them in your Colours of Dance online account.

Terms & Conditions necessary for making a Happy Film in March 2022 - "GOTTA DANCE"

To participate in the Colours of Dance film to be made in March 2022, I acknowledge that I have read and understood the following points:

1. That participation in the Colours of Dance 2022 film is a commitment to
   1. attend a filming session on Sunday 13th March (to be confirmed once responses are finalised) at Comberton Village College
   2. attending normal classes each week to ensure that my child knows the choreography, can be fitted with the costume and is properly prepared
   3. contribute to the costs of putting on this event through the payment of a participation fee (£12) and optionally a copy of the film.
2. That the filming day will be held in accordance with prevailing government pandemic guidelines.  Should the filming have to be cancelled/rescheduled, I understand that Colours of Dance will do their best to provide an appropriate alternative performance/filming experience for my child if that is possible.   If that is not possible, Colours of Dance will refund to me the cost of film downloads I have purchased.
3. That Colours of Dance may be required to apply to Cambridgeshire County Council for a Body of Persons Approval under regulations concerning child performances, and we will share any of the personal information supplied by you only with Cambridgeshire County Council as required to do so by them for this purpose.
4. That other than normal Colours of Dance uniform which may form part of the costume or a base layer, Colours of Dance will source and supply my child(ren)s costumes.
5. I warrant that my child(ren) named in this application is/are fit and healthy enough to take part, and that the theatre environment is suitable for them socially & emotionally.  Should that change, I commit to informing the Principal immediately.
6. I will advise in this application (on subsequent pages) of any special needs or medical details that Colours of Dance teachers and volunteers should know about so that my child may take part and be cared for appropriately (e.g Athsma, allergies, fear of small places/loud noises etc) but which do not preclude them from participation.  I will advise Colours of Dance if a condition arises during the preparation period between completion of this form and the performances taking place, which may affect my child's participation.  Colours of Dance will do everything they can manage to support any child and their parents who wants to take part, and who may just need a little 'extra' or special consideration to make this manageable (e.g. anxiety).
7. That Colours of Dance will make medical/social/emotional information available to the supervising parent for each child only as needed to ensure their safe & proper care and I give my permission for CoD to do this as needed.  
8. That my child(ren) named above will be cared for by a parent or Colours of Dance teacher assigned to them backstage at all times, and not by me.
9. That Colours of Dance will have carried out a thorough Risk Assessment and will ensure a minimum of two trained First Aiders on site at all times at Comberton Village College.
10. I give permission for my child to be filmed and included in the resulting downloadable film or DVD (whichever is produced). I understand that my child’s name will be listed in the credits at the end of the video/in the programme, but not linked directly to their image.
11. That I agree to pay the event fee of £12 per child involved and separately for my copy of the film should I wish to purchase one.  I am aware that I will not be able to view the film without puchasing a copy (anticipated to be no more than £7)
12. That should I withdraw my child(ren) named above after the deadline, I will not be entitled to be refunded the event fee.

Having read and understood the above information and conditions, I agree to abide by them and I consent to my child(ren) named above taking part in the March 2022 Colours of Dance Film.
